1. Confirm you've submitted the Required USAV Documents for your athlete.

2. Look up your athlete's current-season USAV membership number — you'll enter this in the form below.

  • Log in to SportsEngine → Household → your player's profile.
  • Under Overview, click View Memberships, find their current Puget Sound Region Membership ID within in the memberships table.
